Instructions for Submitting Your Program Plan Form

• As of Fall 2023, students are required to complete the electronic program plan form at https://mseforms.ams.jhu.edu.  If you are unable to gain access, please contact Lisa Wetzelberger ([email protected]). Only current students will have access to the site.

1.   Follow the instructions on the Registrar’s website to complete the “Application to Graduate”. This is done through your SIS account. https://studentaffairs.jhu.edu/registrar/students/graduation/.

• If you miss the online deadline to complete the application to graduation, you will need to fill out a pdf version and submit it to SEAM. Download the latest version of the application to graduate here.
• If you are enrolled in the Combined BS/MSE program, and are currently in undergraduate status, you will need to complete the pdf version for the graduate degree.

2.   You must also let the department/program know of your plans. Your program administrator will send out an email each semester asking who plans to graduate. You will need to upload a number of documents, such as a screenshot of your program plan, the certifications to show you completed the Responsible Conduct of Research and Data Science Ethics courses, and more. Fall graduating students can expect this email in September. Spring graduating students can expect this email in February.
