{"id":36782,"date":"2021-09-23T08:37:54","date_gmt":"2021-09-23T12:37:54","guid":{"rendered":"https:\/\/engineering.jhu.edu\/ams\/?page_id=36782"},"modified":"2025-09-17T13:47:45","modified_gmt":"2025-09-17T17:47:45","slug":"ams-mse-student-forms","status":"publish","type":"page","link":"https:\/\/engineering.jhu.edu\/ams\/ams-mse-student-forms\/","title":{"rendered":"AMS MSE Student Forms"},"content":{"rendered":"<ul>\n<li><span style=\"color: #0000ff;\"><a style=\"color: #0000ff;\" href=\"https:\/\/forms.office.com\/r\/z2UzBucqSK\" target=\"_blank\" rel=\"noopener\">Stellic Degree Audit Module Adjustment Request Form<\/a> <\/span><\/li>\n<\/ul>\n<p style=\"text-align: left; padding-left: 40px;\"><em>This form should be used by:<\/em><\/p>\n<p style=\"padding-left: 80px;\"><em>1.\u00a0 Current students submitting a master&#8217;s thesis proposal,<br \/>\n2.\u00a0 Current students requesting review of a <span style=\"text-decoration: underline;\">new<\/span> non-AMS course toward degree requirements, and\/or<br \/>\n3.\u00a0 Current students who have examined their Stellic record, but courses are not appearing as they should.<\/em><\/p>\n<p>&nbsp;<\/p>\n<ul>\n<li><span style=\"color: #0000ff;\"><a style=\"color: #0000ff;\" href=\"https:\/\/mseforms.ams.jhu.edu\/\">AMS Master&#8217;s Program Form<\/a> <\/span><\/li>\n<\/ul>\n<p style=\"padding-left: 40px;\"><em>This form should be used only by current students who need a Stellic workaround, <span style=\"text-decoration: underline;\">and with advance permission<\/span>.<\/em><\/p>\n<p style=\"padding-left: 40px;\"><em><span style=\"font-size: 12pt;\">Tips for Completing the online AMS Master&#8217;s Program Form<\/span><\/em><\/p>\n<p style=\"padding-left: 40px;\"><span style=\"font-size: 12pt;\">We <strong>STRONGLY<\/strong> recommend you submit this form each semester around registration time, and you should estimate your future courses.\u00a0 Students <strong>must not<\/strong> wait until the semester of their graduation to submit their first program proposal.<\/span><\/p>\n<p style=\"padding-left: 40px;\"><span style=\"font-size: 12pt;\">I. If you are pursuing the 8 courses plus thesis option, be sure to send your research proposal to Kristin Bechtel right after completing the online form.\u00a0 They are reviewed together.<\/span><\/p>\n<p style=\"padding-left: 40px;\"><span style=\"font-size: 12pt;\">II. List your focus area in the appropriate field.\u00a0 Your focus area courses should then be listed on the first three lines of the course list.\u00a0<\/span><\/p>\n<p style=\"padding-left: 40px;\"><span style=\"font-size: 12pt;\">III.\u00a0 Do not leave the Academic Ethics and Responsible Conduct of Research fields blank.\u00a0 You should complete both courses during your first semester.<\/span><\/p>\n<p style=\"padding-left: 40px;\"><span style=\"font-size: 12pt;\">IV.\u00a0 Be sure to list your computing course and the semester\/year you completed
